Complete the checklist below for the type of plan you need. All checklists are fillable pdfs which are easy to complete and email to us. As soon as we receive it, we will email you a confirmation. We then use the checklist to complete your plan document, summary plan description, adopting resolution and any administrative forms needed to adopt the plan document. Within 5 to 7 business days, we will email your plan document, SPD, adopting resolution and forms to you in both Word and PDF versions.
